Yes. The online patient form is hosted by Adit, a HIPAA-compliant dental software provider. Your information is encrypted and transmitted securely. It is never shared with third parties and is used solely to prepare for your appointment at Carmel West Dentistry.
Absolutely. The online form is fully mobile-optimized and works on any smartphone, tablet, or computer. Most patients complete it in about 5 minutes from their phone while waiting for their appointment time to approach.
No problem at all. Our team reviews your form before the appointment and can make corrections with you when you arrive. If you submitted the online form and need to update something, just call us at (317) 253-8631 before your visit and we will make a note of it.
The form is currently available in English. If you are more comfortable in Arabic or French — both of which Dr. Tannous speaks fluently — please call us at (317) 253-8631 when booking your appointment and we will make sure the right arrangements are in place for your visit.
Do not worry. We have copies at the office and can give you one to complete when you arrive. Just plan to arrive 10 to 15 minutes early so we have enough time before your appointment begins. Alternatively, you can always switch to the online version and submit it from your phone on the way in.
